If you are interested in being in selected to participate in the study, we ask that you complete the final questionnaire below.Ý From this questionnaire, we will select our final candidates.Ý Please return this questionnaire to us by July *, 2002.

As a reminder, if selected for the Long Term Nutrition Study, your participation would involve: 1) the submission of your weekly grocery receipts, 2) the completion and submission of a questionnaire that reflects your food consumption for the week, and 3) your participation in one interview.ÝÝ You will be provided self-address stamped envelopes to mail your receipts and questionnaires to us.Ý

We anticipate starting the long-term survey in mid-July.Ý It will last a maximum of 3 months.Ý You will be paid $20 for each weekly you submit your receipts and questionnaire in a timely manner (following the survey schedule).Ý Failure to submit your receipts according to the survey schedule could result in decreased compensation or termination of participation.
